Clinical hours log template

A free spreadsheet for recording supervised clinical hours one activity at a time, in the same columns Accrue uses for import and export.

One row per activity: date, activity, duration, site, supervisor, client type, client age, modality and notes. Open it in Excel, Numbers or Google Sheets (File, then Import).

The columns

Columns in the clinical hours log template
ColumnWhat to write
DateThe day of the activity. Write 2026-09-08, 9/8/2026 or 9/8/26.
ActivityWhat you did, such as Therapy, Intake, Individual supervision, Group supervision or Notes. To import into Accrue, use the activity names in your Accrue menus.
Duration (h:mm)How long it took, as hours and minutes: 0:50 for a 50-minute session.
Reported (hours)The same duration in decimal hours, 0.83 for 50 minutes. Optional when h:mm is filled in.
SiteWhere the work happened. Every activity except class time needs one.
SupervisorWho supervised the work at that site.
ClientIndividual, Couple, Family or Group, when the activity had a client. Never a client's name.
Client ageAdult or Minor.
ModalityIn person, Video or Phone.
NotesYour own reminder. Keep client names and clinical details out of it.

Keeping the log

  • Write one row per activity, on the day you do it, with the exact minutes.
  • Keep program and board questions out of the log itself. The same row may count differently toward your degree program and your license.
  • Record supervision as its own rows, with the format: individual, triadic or group.
  • Keep the signed forms your program or board asks for. A spreadsheet is not a signature.

Moving it into Accrue

In Accrue on iPhone, open Hours, tap Export, and choose Import a spreadsheet. Accrue creates the sites and supervisors it has not seen, lists any row it cannot read with the reason, and does not add the same row twice if you import the file again. Each imported row is then counted against your program and board ledgers. Review the new sites and supervisors afterward. See Reports, exports, and signatures.

Common questions

Is the template free?
Yes. Download it and use it in any spreadsheet. You do not need an Accrue account.
Can I bring the spreadsheet into Accrue later?
Yes. Accrue reads this file as it is. Rows it cannot read are listed with the reason, and importing the same file again does not add the same rows twice.
Does a spreadsheet show whether my hours count?
No. A log records what you did. Whether an hour counts depends on your program's and your board's rules, such as supervision each week, caps on some activities and time limits. Accrue applies those rules to the same rows.
Should I record client names?
No. Record the type of client and the format, never a name or clinical detail.
